First I start off with the "Peace of mind" hanger by sold by McKenzie. I made a pattern out of cardboard of the hanger.  I cut the slot a little bigger for the head of the lag bolt to slide up in. I then transfer the pattern to 1/2" plywood and cut it out on the bandsaw. This is better than just screwing to the back of the mount and routering out the hole for the bolt to fit in. When hanging a real heavy mount it's hard to get the lag bolt in the hole for the hanger to slide down on. With the plywood spacer if you can hit the bottom of the hanger anywhere it will slide down on it. I also cut a wooden dowel 1/2'' thick and put one each side on the bottom of the mount to match the offset at the top. I also use the noose hanger cut in half with the same method for smaller mounts. I attched photos of a moose I recently did.
  Some may already use this technique or have a better one. But this works great for me.
