You gotta get off the Cap Lock key. It's tough to read and the forum asks you not to do it (scroll the page down and read that last sentence) Next, you don't have the name correct. Most likely you have a Pere David's Deer. Get a Van Dykes Catalog and you'll find that they DO have such a form in their inventory. Good luck.
Robert, this is not mean't as a slam to you so please don't take it wrong.
A Pere David's Deer is a trophy that few sportsmen even know about, much less shoot one. The only place they can be hunted is on game ranches and you'll pay a hefty chunk out of your yearly salary when you pull the trigger.
I'm amazed that anyone would give such a mount to a beginner to do!

A fool and his money will soon part! Who ever would bring a terrific trophy like a Pere Davids deer to someone that doesn't even know what one is, deserves to get a crappy mount back. I hope you plan on getting some pictures for referance and so forth.
