Chewing?
Chewing with his mouth closed. Maybe getting ready to snort at the photographer? Signs of malnutrition, that's usually in winter. alot of times fawns will have this look, maybe excitement. Deer have alot of weird expressions when captured with a click of a camera shutter.

...a buck that has already lost his neck swell. Cover the sides of his neck with your fingers, to suggest the swell could be there, and he seems fine.

...Keep in mind that it's facing directly at the camera. And you lose your depth perception in the 2D photo as there's not much in the photo that gives our brains a sense of depth. So the puffing of the cheeks may also be accentuated by this phenomenon...
The photo was rather dark on my screen and I thought he looked like he was chewing. I moved the photo to Adobe Photoshop and modified it a little. Gorb is correct, the mouth is not open, so I am throwing chewing out the window. I think my monitor is going. Weird deer Gina.
He's getting his stuff out tomorrow. I will have an attourney appointment on thursday, set up with a nice one. I am pretty confident I can close this mess without too much smoke and flame. LOL
