REGISTRATION RELEASE STATEMENT: I understand that by pre-registering, my convention packet will be ready for me to pick up at the pre-registration desk when I arrive in Lubbock. I understand that once I receive my convention packet, I am totally responsible for all identification badges and tickets for the awards banquet, special seminars, tours, and competition entries. If these are lost, stolen, or misplaced, I understand that I cannot expect any refund or duplicate badges or tickets to be issued to me. The identification badges MUST be worn at all times to enter any NTA convention function or security personnel will refuse entry. If I am forced to cancel my plans to attend the convention, I must request a refund in writing and the following will apply: Postmarked by June 11th, 2001 - A 50% cancellation penalty * After June 11th, 2001 -- NO REFUND.
