What Form Is This??

    Was told it was a McKenzie form but I can’t figure out which one. Kinda looks like a Revolution. Anyone have any ideas?? Customer what’s this exact form. ‍♂️

    It like a 9800 series upright wall pedestal right turn to me.

    Yep I’d say a McKenzie 9800 right turn , take it off the wall there will be a sticker on the back .

    Don't think it's a 9800, the turn is not sharp enough, possible Ohio Tax form.
